Attributed lives and their economics
MA membership, ACO participation, capitation arrangements, and documented quality/risk-score performance — the per-member-per-month story buyers now lead with.
Specialty focus · Primary Care
Value-based care changed what primary care practices are worth — and who's buying. Most valuations haven't caught up. Ours have.
For decades, primary care practices traded modestly: fee-for-service economics capped earnings, and value sat in asset-plus-small-goodwill territory. Value-based care broke that ceiling. Practices managing attributed Medicare and Medicare Advantage lives — earning capitation, quality bonuses, and shared savings — now attract buyers underwriting per-member economics rather than per-visit ones, at valuations fee-for-service thinking can't explain. If your valuation treats your attributed panel as ordinary revenue, it's underpricing your practice. Ours doesn't.
Certified, USPAP-compliant appraisals for sales, buy-ins, divorce, disputes, SBA loans, and estate planning.

Upside-only versus risk-bearing arrangements, payer relationships, and renewal posture. The same panel earns very differently under different contracts.
Medicare-heavy panels carry value-based upside; commercial panels carry fee-for-service durability; the blend defines your buyer list.
Team-based models with NPs/PAs, same-week access, and care management infrastructure make panel economics transferable and scalable.
Visit volume, coding integrity, and ancillaries (point-of-care lab, chronic care management billing) still anchor the base case.
California primary care is being acquired by health systems defending attribution, value-based platforms and enablement companies building risk-bearing networks, medical groups scaling MA panels, and FQHCs expanding access — alongside the perennial physician-to-physician market. Each prices a different aspect of the same practice. Our valuation establishes the defensible base; our brokerage makes the right buyers compete above it.
Sale and platform-offer evaluations; valuations for joining (or exiting) risk-bearing groups; partner buy-ins; concierge/DPC conversion feasibility analysis; divorce and SBA matters.

The earnings base itself. We separate fee-for-service earnings from value-based streams, test each for durability, and value attributed-panel economics the way today's buyers underwrite them. A 2015-style revenue-multiple guess misses the entire repricing.
Documented shared savings history adds real, valuable evidence of panel manageability — buyers pay for proof. Upside-only earnings are valued more conservatively than capitated streams, and the report distinguishes them honestly.
Sometimes dramatically, sometimes not at all — it depends on panel willingness to pay, your market's depth, and execution timeline against your exit horizon. We model conversion scenarios against the sell-as-is baseline so the choice is quantitative.
Often both — equity arrangements, MSO relationships, or full acquisition. Their offers are structurally complex, and evaluating one is precisely the moment for an independent valuation and offer model.
