Normalizing a Cardiology Group's Earnings

Cardiology normalization is layered. Physician compensation restates to subspecialty-specific benchmarks — interventional, EP, and general cardiologists carry very different replacement costs. Ancillary revenue separates by modality (echo, nuclear, vascular, stress, device clinic) with technical and professional components distinguished, because buyers — especially hospitals — value them differently. OBL distributions, imaging equipment leases to related parties, and call-coverage stipends all get restated. Deferred capex gets quantified: a nuclear camera at end-of-life is a liability hiding in your earnings.

Approaches and the Multi-Partner Reality

The income approach capitalizes normalized cash flow at rates reflecting referral durability, payer trajectory for your highest-volume codes, and physician concentration. The market approach draws on cardiology platform and system transactions — a comp set that has deepened rapidly — adjusted for ancillary depth and subspecialty mix. The asset approach matters more here than in most specialties given the equipment base. For partner transactions, we value the specific interest under your shareholder agreements, with control and marketability treated explicitly — cardiology's multi-partner buy-ins and retirements are among our most frequent engagements (see buy-in & buy-out valuation).

Purposes

Platform/hospital offer evaluation; partner transactions; OBL and ASC interest valuations; shareholder disputes; divorce and estate matters. Selling? See cardiology brokerage.

Three Engagement Tiers, One Standard

We scope the right tier on your discovery call, so you never pay for more report than your purpose requires.

Hourly Consulting

Scoped on your call

A second opinion without commissioning a full valuation. Physicians use it for an independent read on a private equity offer, a partner buy-in, or a purchase they are weighing before signing a letter of intent.

A clear, three-step process

  1. 1

    Discovery Call

A confidential conversation about your practice, your timeline, and the purpose of the valuation. We scope the right report tier before you commit to anything.

  2. 2

    Financial Analysis & Interviews

We interview you and normalize your financials. Owner compensation, one-time expenses, and related-party leases all get restated. Typical turnaround: 1–2 weeks.

  3. 3

    Certified Opinion of Value

You receive a defensible, USPAP-compliant conclusion of value, walked through line by line. The report is documented to withstand scrutiny from a buyer, a court, a partner, or a lender.

Frequently asked questions

As its own earnings stream with its own risk: reimbursement trajectory by modality, equipment age and replacement cost, and accreditation posture. Technical-component earnings are the heart of cardiology value — and of hospital buyers' interest — so they're modeled, not blended.

Collections-based formulas ignore profitability and ancillary mix — two partners with equal collections can contribute very different earnings. We're frequently retained to either apply such formulas faithfully or to demonstrate why they should be renegotiated. Both start with an independent valuation.

Contracted stipends count as earnings with contract-renewal risk attached; informal arrangements get probability-weighted. Either way, transferability to a successor group is the question we answer.

Yes. Valuation engagements are handled remotely from our California office. We work from your financials, interview you by phone or video, and send you the finished report. We have completed valuations in more than 35 states, and an on-site visit can be arranged when an engagement calls for it. Many physicians have no local appraiser who focuses on medical practices, so working with a specialist remotely often serves you better than hiring a generalist nearby.
